Please enroll my child in the following session(s). I understand that the cost for each session is based upon the fee schedule below and a $20 one–time registration fee is also required. I also understand that my child cannot be enrolled without a completed application along with payment in full with no refunds allowed, only a change in weeks if space permits.

Fee schedule

Registration fee:
• A $20 one–time fee which will include all sessions

Tuition fee:
• $129 per session/$99 for session 2 if registered by March 31, 2011
• $139 per session/$109 for session 2 if registered by April 30, 2011
• $149 per session/$119 for session 2 if registered by May 31, 2011
• $159 per session/$129 for session 2 if registered after June 1, 2011
